1	Photocopy of your birth certificate. If you do not have one then substitute with a copy of your passport.
2	Photocopies of your high school, OAC, College and or university marks.
3	Your personal essay indicating your reasons for wanting to become a physician.
4	Three color photos, passport size (2" x 2")
5	A bank draft or certified cheque of $100 USD (one hundred) payable to Universities123.com

Since the mails are very slow and because we wish to have security on the materials please courier the above materials.

Later when you receive them also mail or have mailed the following.

1	Two letters of recommendation from a college or university aquainted with your character and abilities.
2	Official transcripts from each high school, college, university or professional school attended.
3	TOEFL scores must be 550. Please call Dr. George K. Lewis (905)835-0966 if you have questions.

Health Certificate/proof of Immunization to include: (rubella, deptheria, tetanus, polio, tuberculin test within past year). If you have a copy, you may fax it to (905) 866-6761. If you do not have this document you may obtain it at Bonaire since it is not required until you enter the U.S. Hospitals after 1 year and 4 months at Bonaire.

Once you have been approved, you may firm your start date so that one of 80 seats per semester is earmarked for you by mailing to the above address a check for $2,000.00 USD (two thousand) made out to Universities123.com.

Fax copy of bank telephone transfer to (905) 866-6761. If necessary call the telephone operator for routing code number.
