You can upload or Mail the following documents to:

1 A completed application form. Your application will not be processed if it is incomplete.
2 Four passport-sized photographs. Please put your name, SIN # and signature on the back of each photo.
3 A non-refundable fee of $100.00 USD. Application will not be processed without the application fee.
4 Three letters of recommendation from professors, employers or a pre-professional committee.
5 All 3 essays typed on seperate sheets of paper. Please ensure all your essays are not single spaced, and are legible.
6 Official transcripts from highschool and university.
7 MCAT is not required, if you have completed the MCATS's or any other tests please send in test results.
8 Curriculum vitae or résumé. references should be included.
9 Letter of good conduct or background check from the police department. Please submit an official letter from the police department. No photocopies.
10 Please complete a health form and have your immunization records forwarded to the school.

Essays

Please complete all the following essays and submit them with your application. Include your name and SIN # on each page.

1 Please discuss your medical career expectations as a fuure physician. (approx. 500 words)
2 What significant accomplishments or life experiences make you unique? (approx. 250 words)
3 Why would you be an asset to the medical school you are applying to? (approx. 250 words)
